Dues for the year 2007-2008 are $200. Please pay by ____________. Any additional dues for the
year are to be decided on by the board of directors, which includes the coaches, and a team/parent
representative.

Uniforms are to be handled by each team. The team may have a fundraiser, acquire a sponsor, or
pay out of pocket. (This is to give the parents more flexibility from scheduled fundraisers)

If this is your first year, please enclose 6 copies of your birth certificate.
